If you want to keep Apollonia alive then either Sonny or Connie has to die to serve as the ultimate impetus for Michael. Fredo dying during the Vito hit wouldn't have made a difference. Tom was the pseudo-civilian and he's not even Italian, so the other families wouldn't give a shit about him.

Both the book and the movies made it clear that the family was in decline when Michael rose to the occasion and became way more ruthless than Vito or even his own personal hitman, but all of those mostly happened because of all the important deaths in his family. If no one died Michael would have just brought Apollonia back to the states and continued his old plan to be a normie despite having murdered 2 people, at least until something inevitably happens to Sonny.
